For non-farm heavy equipment, list the original cost. If a mobile home has been sold, please make a note in the comment box below stating the name and address of the person you sold it to, date sold and where the mobile home is now located.

If more than one person living in your household has a vehicle or other personal property registered in their name, please fill out a separate form for each individual.

Enter any items you did not own as of January 1 this year under "Remove Items".

Important: Any such person who willfully subscribes any such return which he does not believe to be true and correct as to every material matter shall be guilty of a Class 1 misdemeanor (Code of VA - 58.1-11)
